Hiring Integrity Printable Forms
Give each person the worksheet written for their role. Compare the answers only after everyone has reviewed the same job alone.
The download contains six PDFs. The four role-based worksheets are for a candidate; a hiring manager; a recruiting or people team; and an advisor. A two-page group sheet helps the group record disagreements, missing decisions, owners and due dates. The Start Here guide explains how to run the review.
Every worksheet can be filled on screen or marked with pen or pencil. The pages use US Letter sizing, printer-safe margins, readable type and answer marks that remain clear in black and white. A4 users can print at Fit.

A simple group workflow
- Choose one current job. Do not combine several openings or rely on a general company policy.
- Answer alone. Each person uses the worksheet written for their role and marks what they can prove today.
- Compare the gaps. Start with No and Cannot confirm. Then discuss answers that differ across the group.
- Assign the repair. Record the missing decision or fact, who owns it, the due date and what candidates need to be told.
Use the interactive scorecard when one person wants explanations and next steps on screen.
Sources, limits and use
The interview questions draw on the U.S. Office of Personnel Management's guidance on structured interviews. The accommodation questions draw on the U.S. Equal Employment Opportunity Commission's guidance for job applicants. The questions about automated hiring tools draw on the NIST AI Risk Management Framework.
The scorecard is not legal advice or a validated hiring test. It cannot prove that a company broke the law or that a hiring decision was fair. It reviews what one person can see or show about one job.
